10/25/2023 EnchantaVaughn Approved Behavior: QMHP observed client and his mother and they appeared to be in a pleasant mood as demonstrated by the smile on their faces. Client was attentive and eager to participate in the session, as he grabbed his sneakers and jacket to sit with his counselor on the porch while his mom and siblings remained in the house finishing other house duties. Client displayed appropriate hygiene (clean face and fresh haircut), and his mood was receptive to receiving counseling at this time. Behavior: During the session the client was alert and focused. The clientâs behavior was acceptable as he was respectful and cooperated with the counselor when prompted to complete his therapeutic work. Client and QMHP interacted in a safe and positive manner. Client was able to learn new skills to implement in his daily routine and appeared to be showing more effort at home. Mom communicated her thoughts and feelings well regarding his recent behavioral performances and reports. Intervention: QMHP facilitated an individual session with client on positive social interactions with his family and peers. QMHP conducted this intervention with client on building positive relationships daily. The QMHP educated client that positive social interactions are interactions that are positive in nature and successful for both people involved. QMHP informed him that peer interactions are important because they lead to positive social and emotional development in children with ADHD and other mental stressors. Intervention: QMHP also informed client that children, who can interact successfully with their peers, have stronger friendships, and are included more often in activities than children who lack these skills. QMHP also explained to client that social interaction builds friendships that promote positive social and emotional development at home, in school, and in the community. QMHP observed client displaying positive social interaction with mom and establishing a healthy relationship with his siblings after the interventions were processed. Response: Client was observed and reported eliminating personal conflicts at home and school. Client utilized conflict management skills at home, school, and in the community. Client exhibited techniques for appropriate self-expression and active listening. Client expressed a belief in self as being lovable and capable. Client demonstrated appropriate interpersonal skills, assertiveness, confidence in self and ability to problem-solve on his own. Response: Client participated and cooperated well with the social skills activities. Client was seen completing individual assignment. Client was able to answer the thinking questions with assistance from QMHP. Client asked appropriate questions concerning the social skills activities. Client communicated positive self-talk and affirmations. Client was observed complying to authority requests and demands the first time they are given. Client was able to recognize personal strengths and abilities to perform better at home.
